This is the TC-1 Automatic Turnout Control from Circuitron.

Features

Activates dual-coil switch machine when used with DT-4. Momentary pulse when detection unit is activated, protects from burnout. Can be used to control both directions. Requires 10-18V AC or DC input. The higher the voltage, the greater the output power. Recycle time on each side is 1-2 seconds.
